Physicals: The Physical Form* should be emailed to records@mchs.org.  This form must be completed by July 1, 2022.

*Any exemption requests for immunizations or examinations due to religious reasons.  Parents must complete this form.

Uniforms: August through September students will be in Warm Weather Dress Code which consists of a Mount Carmel polo shirt, khaki dress shorts (no cargo pockets), a black or brown belt, socks and gym shoes. On October 1st, students will switch to Khaki dress pants, a brown or white Mount Carmel polo, a brown or black belt, socks and all-black or all-brown shoes. As the weather grows colder, students may wear a non-hooded MC jacket or sweatshirt over their polo, or a long-sleeved Mount Carmel shirt.  Hooded sweatshirts are not allowed and will result in a dress code violation.  On Mass days, usually once a month, Mass Dress Code is required.  Mass Dress Code consists of dress pants, a white or solid blue dress shirt, sport coat, tie, belt and black or brown shoes.
All MC polos and t-shirts are purchased, either online or in-person, at the Spirit Shop. Spirit Shop hours during the school year are Tuesdays and Thursdays 10:00 am to 12:30 pm.  The Spirit Shop will be open during Freshmen Orientation and during the summer.  Stay tuned for summer hours.

Bring Your Own Device: The selected device must have at least a seven-inch screen, a physical keyboard, the ability to cut and paste, and a 3 ½ hour battery life. Any device that meets these standards will be permitted.  Phones are not permitted in place of a device at any time.  For more information, click here.

Books: Instructions on purchasing text books will be emailed to you during the summer with the booklist and links to the online bookstore.  Text books are available in print and some offer an electronic option.
